PARENT CONSENT & LIABILITY WAIVER
Artberry Creative Studio / Artberry LLC
Please read this Parent Consent & Liability Waiver carefully before registering your child for an Artberry Creative Studio class, camp, workshop, or other program.
1. Permission to Participate
I am the parent or legal guardian of the participating child and give permission for my child to participate in programs and activities offered by Artberry LLC, including weekly art classes, camps, workshops, and related activities.
2. Acknowledgment of Risks
I understand that participation in art and studio activities involves ordinary risks. These may include, but are not limited to, minor cuts, scrapes, splinters, slips or falls, contact with paint, glue, clay, art materials, scissors, tools, furniture, or equipment, and possible allergic or skin reactions to materials.
Some programs may also include supervised outdoor activities, snack or lunch breaks, walking in areas immediately surrounding the studio, or outdoor games.
I understand that reasonable precautions will be taken to provide a safe environment, but not every risk can be eliminated.
3. Assumption of Risk
I voluntarily allow my child to participate and accept the ordinary risks associated with participation in Artberry programs and activities.
4. Release of Liability
To the fullest extent permitted by applicable law, I release and hold harmless Artberry LLC, Artberry Creative Studio, its owner, employees, contractors, and representatives from claims arising from the ordinary and inherent risks of participation in Artberry programs, including claims arising from ordinary negligence.
This release is not intended to waive any rights or claims that cannot legally be waived under applicable law, including claims arising from gross negligence, reckless conduct, or intentional misconduct.
5. Health, Allergies & Special Considerations
I understand that I am responsible for providing accurate and current information regarding my child’s allergies, medical conditions, accommodations, behavioral needs, or other circumstances that may be important for safe participation.
I will notify Artberry of any relevant changes before my child’s next class or program.
I understand that Artberry Creative Studio is not a medical facility and does not provide medical diagnosis or treatment.
6. Emergency Medical Authorization
If my child becomes ill or injured during an Artberry program, I authorize Artberry to contact me and the emergency contacts provided during registration.
If I or another emergency contact cannot be reached and immediate medical attention is reasonably necessary, I authorize Artberry to contact emergency medical services, including 911, and to take reasonable steps to obtain emergency care for my child.
I understand that I am responsible for medical expenses resulting from such care.
7. Outdoor Activities
When applicable to the program, I give permission for my child to participate in supervised outdoor activities, snack or lunch breaks, and games in areas near Artberry Creative Studio.
I understand that outdoor plans may be changed or canceled because of weather, safety concerns, or other circumstances.
8. Behavioral & Safety Expectations
I understand that children are expected to follow reasonable safety instructions and behave respectfully toward other students, instructors, materials, and the studio.
If my child’s behavior presents a significant safety concern or seriously disrupts the program, I understand that I may be contacted and asked to pick up my child.
9. Personal Belongings
I understand that Artberry is not responsible for lost, stolen, or damaged personal belongings brought to the studio or to related activities.
10. Parent/Guardian Certification
By agreeing below, I confirm that:

I am the parent or legal guardian of the participating child, or I am otherwise legally authorized to provide this consent.
